Tamoxifen is a selective estrogen receptor modulator because it binds to estrogen receptors and blocks estrogen signaling in breast tissue, acting as an antagonist there. This prevents estrogen-driven growth of ER-positive breast cancer cells, which is why it’s used as adjuvant therapy and for prevention in high-risk women. In other tissues it can act as a partial agonist, explaining some side effects like endometrial effects and hot flashes. This distinguishes it from aromatase inhibitors, which lower overall estrogen levels; from estrogen receptor agonists, which would stimulate estrogen signaling in the breast; and from androgen receptor antagonists, which target a different hormonal pathway.
